“My computer was having this problem, when I'd wake it up from sleep mode, everything would be all blurry and pixelated like it was set to a zoomed in resolution (although it's not). I would have to refresh the resolution or turn the monitor off and back no to fix it.
I just knew that it would eventually get worse to where I couldn't fix it, and those tricks wouldn't work. Well, I was right. Now, I am stuck with everything choppy and pixelated looking, even as I type this. Refreshing or changing the resolution does nothing. I've unplugged and replugged the hdmi cable and that did nothing.”

By PCSwizz: “At this point, it would be difficult to figure out if either your graphics card is overheating/bad, or your monitor is the problem.
 
You mentioned that you turned your monitor off and on to fix it before, so I would assume the monitor is faulty. Try replacing the monitor and see if that helps. If you still are facing this issue after replacing the monitor, then the graphics card on your computer is either overheating or faulty.”
